Reporting Incidents

The Reporting Incidents section in Clarity AI is used to create a new incident record when an issue related to AI systems, datasets, or governance processes is identified. 

This process ensures incidents are documented in a consistent format so they can be reviewed, tracked, and managed through the Incident Management Dashboard. 

Purpose 

Reporting an incident captures important information about an event that may impact AI system behaviour, data handling, compliance obligations, or operational processes. 

Creating an incident record ensures the issue is formally logged and available for review and follow-up actions. 

Information Captured 

When reporting an incident, the following details are recorded: 

  • Incident Title – name used to identify the incident 
  • Description – summary of the issue and relevant context 
  • Incident Type – category used to classify the incident 
  • Severity Level – impact classification such as Low, Medium, High, or Critical 
  • Detected Date – date the incident was identified 
  • Related AI System or Dataset – associated system or data asset, where applicable 
  • Owner or Assignee – person responsible for reviewing or managing the incident 

These fields ensure the incident is recorded in a structured and consistent format. 

How Reporting Works 

Selecting Report Incident opens the incident creation form. 

Users enter the required information and submit the form to create the incident record. 

Once submitted: 

  • the incident becomes visible in the Incident Management Dashboard 
  • the incident status is recorded 
  • the incident can be reviewed and updated as required 

Notes 

  • Incident records should contain clear and accurate information 
  • Severity levels should reflect the potential or actual impact of the issue 
  • Linking the incident to the relevant AI system or dataset ensures traceability 
  • Incident details can be updated as additional information becomes available
